This form is to be used by participants of the Cure Leukaemia Tour 21.

Please can all participants complete the form with as much information as possible, so that our event crew can be prepared for mechanical situations and so that we can ensure the correct spares are provided.

IMPORTANT: Please remember to bring along your own specific spares as well i.e. spare spokes for your wheels, shoe cleats, tyres and tubes. If you aren’t sure what spares to bring, please see your kit list.
